Application of the diagnosis of health in the neighborhood September 25. Canton La Libertad, 2016

Abstract

The present article has as main objective to improve the quality of life in patients suffering from diabetes mellitus and arterial hypertension in the neighborhood of September 25, canton la liberty in the province of Santa Elena. The application of the diagnosis of health is a necessary practice for the doctor and nurse of the family whose purpose is the identification and definition of priorities in the health problems of an assigned community. A descriptive investigation was carried out with search of the primary and secondary information obtaining the methodological selection with contributions of the different actors of the community carrying out the processing, analysis, identification and diffusion through the elaborated survey that allowed us to carry out a deep analysis of the subject Arousing the interest in their residents to know their health condition and how to improve it. So, it is concluded that the health team must identify in a timely manner the problems and risk factors of each of its members and the family offering comprehensive care, promoting self-care, providing support and follow-up, educating, empowering practices and healthy relationships.
